Small-time con artist Randle Patrick McMurphy feigns mental illness to avoid hard labor on a prison farm and is transferred to a psychiatric hospital for evaluation. His free-spirited approach soon clashes with the restrictive regime of the ward, ruled with an iron fist by the head nurse. Their battle of wills soon affects the entire ward.
